The Internal Revenue Code provides several complex rules relating to the taxation of retirement plan distributions. This page contains important information you will need before you decide how to receive distributions.
Taxable distributions are subject to federal tax withholding based on the type of retirement account and type of distribution you elect. Please review the Mandatory Election for Tax Withholding section of the appropriate form(s) below.
IRA Distribution Request Form — Use this form to request a distribution from a Traditional IRA, Roth IRA, SEP IRA, SIMPLE IRA or SARSEP for which Fiduciary Trust International of the South ("FTIOS") serves as custodian.
403(b) Plan Distribution Request Form — Use this form to request a distribution from your 403(b) account for which Fiduciary Trust International of the South ("FTIOS") serves as custodian. Please note: Generally this form must be certified by your employer or your plan’s third party administrator.
Beneficiary Distribution Request Form — Use this form to request a transfer and/or distribution of retirement assets to the beneficiary of an IRA, SEP IRA, SIMPLE IRA, SAR-SEP or 403(b) account for which Fiduciary Trust International of the South ("FTIOS") serves as custodian.
You may wish to consult with the Plan administrator or your tax advisor, before taking a distribution from the Plan. Also, you can find more detailed information on the federal tax treatment of employer plan distributions in IRS Publication 575, Pension and Annuity Income; IRA distributions in IRS Publication 590-B, Distributions from Individual Retirement Arrangements (IRAs); and 403(b) distributions in IRS Publication 571, Tax-Sheltered Annuity Plans [403(b) Plans].
